Proof Check List for KidsCare & AHCCCS
All applicants must complete an application and send proof of information given on the application. Applications must be complete to be processed. Please include all proof that applies to your family.

Wages: |
Copies of check stubs or a statement from the employer showing gross earnings last month and this month
|
|
Self Employment:
|
Copies of current Federal tax forms |
|
Child Support:
|
Copies of the court order or child support payment history |
|
Other income: |
Proof of other income or money received this month. This includes letters from the Social Security Administration, Veterans Adminstration, Railroad Retirement, or other retirement or disability pension. |
|
Health Insurance:
|
Copies of insurance identification (ID) cards for persons who are applying and are currently covered by other health insurance |
|
Citizenship:
|
Birth certificates, U.S. Passport, naturalization certificate |
|
Immigrant Status:
|
Copies of front and back of immigration documents for persons who are applying |
|
Daycare:
|
Proof/copies of the amount billed for the care of a child or incapacitated adult |
|
Pregnancy:
|
A signed letter from your doctor or nurse giving the expected date of delivery |

Identity:
|
Driver license, school picture ID, other government ID, etc. |
