The Applicant Follow Up Plan is one of the most important aspects of the Love Your Kids (LYK) event.
To ensure each family we speak to on February 14, 2008 completes the application process, LYK with AHCCCS, has developed a Follow Up Plan. Login to the downloads section of this site to download the Contact Information Log. Community Outreach Partners and/or Community Health Centers will contact each family to follow up with their completion of the application process.
Follow these easy steps:
1. Record the contact information of each person you hand out an application to on February 14, 2008 using the Contact Information Log.
2. Fax: Fax the document to AHCCCS: Attention Community Relations 602.252.6536
